Client Intake
Booking Policy
- Arrival - Please arrive 5 to 10 minutes prior to the start of your appointment. If it is your first visit, please complete the intake form prior to your appointment.
- Late Arrival - My work is reservation based, so I may or may not be able to accomodate your full or partial appointment time if you arrive late. You will still be charged full price for the service as it was booked, even if you do not recieve your full time. If I have the availability following your appointment, I am generally happy to accomodate you, but it is up to my discretion.
- Age of Consent - You must be at least 18 years or older in order to receive treatment, and be able to provide valid ID if necessary.
- Scope of Practice - Florida Licensed Massage Therapists do not diagnose or prescribe for medical conditions. I may refer you to a medical doctor for diagnosis or treatment. Services are not intended as an alternative for proper medical attention for specific conditions. Please refer to a medical doctor for any specific condition which requires medical treatment.
- Client Needs and Boundaries - I am happy to adjust pressure, temperature, musical volume, work longer on an area or move on if you request it. The client may choose to: leave on as much clothing as needed for comfort, refuse any massage methods, and stop massage at any time. The therapist may also refuse specific massage methods or stop massage at any time, for any reason. Florida Law requires the Breasts, Genitals, Gluteal Cleft and Anus remain covered at all times. The client will always be modestly draped and only the area being massaged will be undraped. Sexual interaction or discussion of any kind between the client and the massage therapist is never appropriate.
- Confidentiality and Conversation - All client visits and conditions are confidential, and protected under HIPPA guidelines. As such, I will not discuss your visit or provide records to anyone without your express permission. You may choose to talk or not talk during the massage. Conversation will be guided by the client's direction.
- Medical Conditions - It is the responsibility of the client to keep the massage therapist informed of any medical treatment currently being taken, and to provide written permission from the physician, chiropractor, physical therapist, etc., that the massage may be continued. The client must also keep the massage therapist informed of any changes in health conditions.
- Hygiene - I do not have a shower on premises, so please make sure to bathe prior to coming in for treatment. I may refuse treatment without refund if you arrive to your service excessively dirty. Please keep this policy in mind if you intend to come for treatment after doing yard work, going to the beach, or coming directly from the gym. Please also refrain from using strong smelling perfumes, colognes, or body sprays prior to treatment.
Cancellation Policy
If you need to cancel or reschedule your appointment, I ask that you please provide at least 24 hours notice before the appointment time for table massage, and at least 1 hour before the appointment time for chair massage. Failure to appear for your appointment ("No-Show"), or failure to cancel your appointment before the designated period, may result in a 25% cancellation/no-show fee to be charged, to the card on file, at the therapist's discretion.
Sometimes emergencies happen - in the rare circumstance that you are unable to make it to your appointment due to an emergency, please contact me as soon as possible, to determine if an exception is appropriate. I am also generally happy to accomodate same-day rescheduling at no charge, assuming that I have availability at the requested appointment time.
